First Visit Guide

Before coming for your first appointment, please check with our staff about the location of the consultation rooms as I consult in three different places.

The first consultation will take at least 30 minutes. I will take a history, review your health background, and perform a physical examination. I recommend that you make a list of your questions to make sure they have all been answered.

If you would like to read about your condition before the consultation, please email us and explain your conditions, and we will send you the relevant material to read. Although this reading will prepare you for the first consultation, it is not a substitute for a full assessment and discussion.

Please remember to bring the following:

• Your referral letter
• Your Medicare, DVA, or pension card
• Your imaging, including your mammogram, ultrasound, and MRI.If you don’t have a hard copy of your imaging, please make sure you know the name of the radiology practice so we can access the online images.
• Your blood test results. If you don’t have your blood test results, please make sure you know the name of the pathology practice so we can access the online results.
• Any other relevant reports or letters related to your medical and family history and your current medications and allergies.
